Hemogram (CBC + ESR)

Hemogram (CBC + ESR)

A Hemogram (CBC + ESR) counts the blood cells and measures the ESR, a general clue to inflammation (the body's reaction to injury or infection). It can be run for dogs, cats, cows and buffaloes.

What this test checks

  • Red blood cell count
  • Haemoglobin and packed cell volume (PCV)
  • Total white blood cell count and white cell types
  • Platelet count
  • ESR: how far the red cells sink in one hour

Benefits of this test

  • Gives two checks, cell counts and ESR, from one blood sample.
  • Adds a general clue about inflammation that the CBC alone does not give.
  • Can help follow a long illness when it is repeated over time.
  • Helps your vet decide whether more specific tests are needed.
  • Needs one tube of blood and gives one report.

When your vet may advise it

  • Fever with no clear cause.
  • A slow illness with weight loss, low energy or poor appetite lasting weeks.
  • Your vet suspects a long-standing infection or inflammation.
  • Follow-up checks during a long illness to see how the animal is doing.
  • Pale gums or weakness along with fever.
  • A dairy animal with a fever that keeps coming back.

About this test

In this lab, a Hemogram means a Complete Blood Count plus an ESR in one test. The CBC counts the red cells, white cells and platelets (small cell pieces that help blood to clot) and measures haemoglobin, the oxygen-carrying protein in red cells. The ESR adds a simple check of how fast red cells settle in a tube of blood.

For the ESR, the blood stands in a thin tube for one hour. The lab then measures in millimetres how far the red cells have sunk. When there is inflammation, some blood proteins rise and make red cells stick together, so they settle faster. Anaemia, which means too few red cells, can also speed up settling.

Vets use the Hemogram when they want the full cell picture and a general marker of inflammation, for example in a long fever or a slow illness. ESR is only a clue. It does not name the disease.

How to prepare

No fasting is normally needed, but follow your vet's advice. Ask your vet to draw about 2 mL of blood into an EDTA tube (purple top), and get the tube to the lab quickly, because ESR needs fresh blood. Keep it shaded and unfrozen; the report is normally ready the same day or within 24 hours.

Reading the result

The CBC values and the ESR are read together. A raised ESR may indicate inflammation or anaemia but cannot name the cause, so a registered veterinarian uses it along with the animal's signs and other test results.

Questions about Hemogram (CBC + ESR)

What does ESR mean?
ESR means erythrocyte sedimentation rate. It shows how fast red blood cells settle at the bottom of a tube in one hour. A faster rate may point to inflammation, but it does not tell your vet which disease is present.
Is a Hemogram different from a CBC?
Yes, a little. The Hemogram includes everything in a CBC and adds an ESR. If your vet needs only the cell counts, a plain CBC is enough. Ask your vet which one is needed for your animal.
Can a normal ESR rule out illness?
No. A normal ESR does not rule out illness, because early or mild disease may not change it. Vets read ESR together with the animal's signs, the CBC values and other test results.
Why must the blood be fresh for a Hemogram?
Blood changes if it sits too long, and this can alter the ESR and the cell counts. Bring the tube to the lab the same day, kept in shade and not frozen, so the result reflects your animal's real condition.

Before you book this test

Why this notice: so you know, before you book, how a test is done, what its limits are and who is responsible.

  • Where it is tested. Some tests are done wholly or partly by an independent partner laboratory. The lab tells you before booking if this test is sent out, and the report names the laboratory that did it.
  • What a result can and cannot tell you. No test is right every time. A normal result does not rule out every disease, and an abnormal result does not prove one. A registered veterinarian should read the report with the animal’s signs and history before any treatment is started, changed or stopped.
  • The sample. A sample that is clotted, too small, wrongly labelled, leaking or too old may be rejected or reported with a note, and the lab may ask for a fresh one.
  • Reporting time. Same day to 24 hours. Times are estimates, not guarantees, and the lab confirms the time when you book.
  • Emergencies. A booking or a pending report must not delay urgent care. If an animal has collapsed, struggles to breathe, has fits or bleeds heavily, call a veterinarian at once.
